  3. 2 clinical references or letters of recommendation

  4. copies of your professional licenses and certifications (bls, acls, pals, state license, ARRT, etc.)

*** Please note when uploading documents *** Only documents that are relevant to you need to be uploaded. You do not need to upload everything listed in order to submit. In addition, if you do not have the capability to alter documents, please don't hesitate to upload anyway. Our team is happy to make the necessary changes to your file.

 

Example: Below it asks for the front and back of your BLS to be uploaded separately. Please do not feel the need to separate them if this is an inconvenience. We are happy to make the changes once they have been uploaded.
